The Transition Specialist is the primary resource for inpatient care management, supporting patients in need of post-acute services and transportation. Collaborates with case managers, providers, and post-acute service representatives to determine appropriate care services for patients transitioning from acute settings to home or post-acute care facilities. Responsible for arranging, documenting, and following up on post-acute care and transportation needs.
