The Project Manager II will plan, monitor, and oversee projects related to business development, implementation, and operations in a fully remote role. Responsibilities include managing project life cycles, utilizing project management tools, maintaining project documentation, and communicating project status to stakeholders. A Bachelor's degree and 3+ years of project management experience are required, along with proficiency in project management tools and healthcare experience preferred.

Conduct screening and risk assessment interventions, assist with member care, educate providers and community resources, develop care plans, and create education materials. Requires a Bachelor's degree and 2+ years of experience managing social/community needs in a managed care environment.
