The Director, Community Operations is responsible for managing and developing the community care model, overseeing leadership teams, partnering with clinical operations, developing partnerships, tracking performance, leading recruitment, and collaborating with the leadership team. The role requires exceptional interpersonal and communication skills, organizational skills, and project management abilities.

The Director, Actuary will play a key role in managing the financial risks associated with value-based care constructs for individuals with serious mental illness. They will analyze healthcare claims data, develop financial projections, work with business development and customer success teams, and collaborate with leaders across the organization.

The Community Outreach Specialist is responsible for developing and implementing strategies to engage hard-to-reach individuals living with Serious Mental Illness, and ensuring a warm hand-off to a peer support guide.

Collaborate with clinical services team to provide behavioral health diagnosis and treatment to patients with serious mental illness. Deliver counseling services, suicide crisis support, maintain documentation, and work collaboratively with the team. Must have experience with serious mental illness, telehealth services, and hold relevant licenses and designations.

The People Partner is a key leader within the market community, fostering an excellent culture, strengthening employee relations, and supporting Community team leaders. The role involves collaborating closely with leaders, leading employee relation investigations, creating communications and training materials, and supporting teams with payroll and benefits administration.

Perform community outreach and engage with individuals living with Serious Mental Illness (SMI) to provide support and guidance. Coordinate care services and advocate for personal goals. Provide coverage for after-hours calls on a rotating schedule.
