Responsibilities:
- Assists the Bed Census Management Nurse and Nursing Supervisor with coordinating patient transfers from outside facilities, ensuring appropriate placement based on level of care.
- Facilitates physician-to-physician communication throughout the patient transfer process.
- Collaborates with interdisciplinary healthcare team members to coordinate efficient and timely patient transfers.
- Monitors transfer timelines, maintaining awareness of delays and providing timely status updates to all stakeholders.
- Serves as a liaison between UHS Hospitals and referring/sending facilities to ensure clear and consistent communication.
- Provides high-quality customer service via phone, electronic communication, and in-person interactions.
- Partners with Physicians, Nursing Supervisors, and external facilities to accurately complete Transfer Center documentation.
- Collects, enters, and maintains Transfer Center data to support reporting and operational tracking.
- Adheres to established organizational policies, procedures, and regulatory standards.
- Performs other duties as assigned in support of departmental and organizational needs.
Education/Experience
Minimum Required:
- High School Diploma, 1 year experience working in a healthcare related field.
Preferred:
- Associates Degree
License/Certification
