There is more than one location associated with this position. This is either because there are multiple openings at different sites, or the successful candidate will be required to float across multiple UHS facilities. $19.37 - $28.09 per hour, depending on experience

Responsibilities:

  • Assists the Bed Census Management Nurse and Nursing Supervisor with coordinating patient transfers from outside facilities, ensuring appropriate placement based on level of care.
  • Facilitates physician-to-physician communication throughout the patient transfer process.
  • Collaborates with interdisciplinary healthcare team members to coordinate efficient and timely patient transfers.
  • Monitors transfer timelines, maintaining awareness of delays and providing timely status updates to all stakeholders.
  • Serves as a liaison between UHS Hospitals and referring/sending facilities to ensure clear and consistent communication.
  • Provides high-quality customer service via phone, electronic communication, and in-person interactions.
  • Partners with Physicians, Nursing Supervisors, and external facilities to accurately complete Transfer Center documentation.
  • Collects, enters, and maintains Transfer Center data to support reporting and operational tracking.
  • Adheres to established organizational policies, procedures, and regulatory standards.
  • Performs other duties as assigned in support of departmental and organizational needs.

Education/Experience

Minimum Required:

  • High School Diploma, 1 year experience working in a healthcare related field.

Preferred:

  • Associates Degree

License/Certification

Preferred:

  • EMT, MOA
