Program Manager III SDoH

Health Net of California and Centene Corporation are in search of an experienced Program Manager III, Social Determinants of Health (SDoH) that will be tasked with helping to develop, implement, and improve our members experiences. This role will be more than standard process improvement. This role will be engaging with our community partners, (local, state, public, private along with internal partners as well) to monitor performance, provider consultative support, and to develop tools and training to ensure Enhanced Care Management (ECM) and Community Support (CS) program success.
Preferred experience includes knowledge and experience developing and/or managing programs in any or a combination of the following CalAIM program areas:
• Enhanced Care Management (ECM)
• Services to Address Homelessness & Housing (Housing transition, housing tenancy, housing deposits);
• Recuperative Services (Medical respite/recuperative care, respite services, short-term post-hospitalization housing, sobering centers); Services for
Long-Term Well-Being in Home-Like Settings (Asthma remediation, day habilitation programs, environmental accessibility adaptations/home
modifications, medically-tailored meals/medically supported foods, nursing facility transition/diversion to assisted living facilities and/or to a home,
personal care and homemaker services)
• Experience working with CalAIM adult and children's populations of focus (e.g., WPC/HHP members; homeless; high utilizers, serious mental illness/substance use disorders; individuals Transitioning from Incarceration)
Additional responsibilities may include:
• Develop strategies to realize improvement opportunities, and ensure organizational prioritization and resource alignment
• Manage multiple projects through full project life cycle process including requirements gathering, creation of project plans and schedules, obtaining and
managing resources, managing budget, and facilitating project execution, deployment and closure
• Provide leadership and effectively communicate project status to all stakeholders, including executive summaries and presentation
• Negotiate with project stakeholders to identify and secure resources, resolve issues, and mitigate risk
• Lead cross-functional meetings with various functional areas to meet overall stakeholder expectations and company's objective
• Provide functional and technical knowledge regarding overall program requirements and operation

Qualifications:
Education/Experience:
- Bachelor's degree in Business Administration, Healthcare Administration, related field, or equivalent experience. Master's degree preferred.
- 5+ years project implementation, product or program management experience.
- Managed care or prescription benefit management experience preferred.
