About our Team: 

We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.

About the Role:  

In this role, you will lead Cityblock’s outreach strategy and execution in Ohio. You will manage the team responsible for locating and engaging the thousands of members assigned to Cityblock, who often face economic and social challenges in addition to having mental and physical health needs. The primary methods of doing this currently are via phone calls, network building, and in-community work. 

You will directly manage a large (10+) team of Outreach Leads and Specialists, each of whom are expected to make hundreds of calls and site visits each week. You’ll work to ensure these contacts are consistently empathetic, persuasive and efficient. You’ll also represent Cityblock externally, building relationships with health and social care partners in the community who can support our member engagement. You’ll also work extensively within Cityblock, representing your organization to Cityblock’s leaders in its NYC-based headquarters, and as a key member of the Ohio leadership team.

You will constantly be reinventing the way that your team operates to ensure that it can succeed as it grows rapidly, within Ohio.

Requirements for the Role: 

  • You have 3+ years of experience managing large (10+) sales/engagement teams, including hiring and ongoing performance management
  • You have experience with community outreach, especially with difficult-to-reach individuals
  • You take full accountability for, and are highly driven toward achieving weekly and monthly performance targets. You can motivate a team to be as driven as you are. 
  • You have extensive experience using member, customer and performance data to improve your and your team’s efforts
  • You excel at empathy and human interactions, and want to improve the health of individuals and whole communities.
  • You are an independent self-starter and a strategic thinker who is eager to impart your experience your team
  • You are excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care

How We Define Success: 

  • Achieve your aggressive weekly and monthly member engagement targets (i.e. how many attributed Cityblock members have engaged in work with our assessment and care teams.)
  • Lead a motivated and high-performing team, who are adept at outreach across several modalities. 
  • Ensure 100% compliance with regulatory requirements
  • Continually improve existing outreach methods, and innovate new ones, to develop a best-in-class engagement operation
  • With the Community Engagement Manager, develop and sustain a highly diverse network of community partners who will help Cityblock locate and engage with its members. This includes hospitals, community-based-organizations and external care navigators.

Nice to Have, But Not Required: 

  • Existing networks within the healthcare space
  • Experience working with individuals with mental health and substance use diagnoses
  • Training in motivational interviewing, behavioral activation therapy, or problem solving treatment
  • Direct familiarity with clinical settings
  • Multilingual (especially Spanish)
