As an extension of the primary care physician’s (PCP) care team, the Triage RN, Shared Services are responsible for providing telephone assessment of patient’s needs. This requires the application of a variety of procedures, policies, and/or precedents relating to both the standards and procedures of the nursing profession and those established by VMD. The triage nurse provides advice and follow-up education consistent with the patient's age and cultural needs. The telephone triage nurse responds to the patient’s inquiries, prioritizing responses and routing urgent situations to the appropriate VMD provider. The telephone triage nurse maintains accurate and appropriate documentation and follows patient privacy in accordance with HIPAA and VMD policies and procedures. The telephone triage nurse performs to the level of a licensed registered nurse as dictated by the Texas Nurse Practice Act and VillageMD.

How you can make a difference

  • Manages patient health concerns and symptoms via a telephone interaction utilizing physician-approved protocols, and online assessment tools (Clear Triage) for the assessment and management of a range of symptoms.
  • Assesses and prioritizes patients’ medical needs via telephone and directs patients to appropriate and necessary care.
  • Demonstrates best practices and quality care.
  • Assesses the physical, social, and emotional needs of patients and families via telephone interactions.
  • Teaches and counsels patients and families concerning heath assessment and care including prevention of disease and health maintenance.
  • Accurately and completely reports assessment, intervention, and/or recommendations provided to patient and/or other healthcare providers.
  • Assist patients and other healthcare entities with inquiries regarding changes in patient’s health status, and accurately and completely performs follow-up activities including providing information on critical laboratory results and diagnostics, scheduling same-day appointments, and assisting with medication inquiries.
  • Calls pharmacy with prescriptions ordered by the provider based on an acute need.
  • Participates in orienting, training, and guiding team members.
  • Guides evaluation including instructing patient/caller how to evaluate normal/abnormal symptoms, effectiveness of treatment and when to call back.
  • Documents symptoms/complaints, nursing assessment, advice and patient/caller response.
  • Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients/callers and their access to appropriate health care.
  • Collects data and make assessments, develops a working nursing diagnosis, determines interventions and disposition per guidelines.
  • Educates patient and/or family members about diagnostic procedures, medications, nutrition and maintenance of health and wellness.
  • Responds to patient questions and appropriately provides reassurance while responding to patient concerns.
  • Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying service excellence, care, concern, respect, and cooperation.
  • Documents all clinical interventions in applicable EMR and electronic software
  • Develop and maintain effective professional working relationships the providers and staff at VMD.

Experience to drive change

  • 5+ years of direct, clinical experience in Medical Surgical Nursing
  • 2+ years of experience in ER, ICU, IMU, Pre/Post-Op, or other high acuity areas
  • Registered Nurse with licensure in the state of practice
  • Telephone Triage experience in a setting that requires assessment, critical thinking and application.
  • Comfort with technology including Microsoft suite of products
  • Utilizing a variety of electronic health records including data capture, data mining and reporting
