Social Services Specialist II
The Social Service Specialist II is a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community. The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning. They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs. Possess a clear understanding of discharge and transition planning and linkage with community resources. Demonstrates skills in planning, organizing, and managing multiple functions and complex processes. Required Experience includes 6 months Internship or 1 year Social Work in an acute care or hospital setting.
