Job Description:
Description: The Service Associate is responsible for filing, pulling, and maintaining patient information of medical records and performs other functions regarding medical record/health information management functions.
Typical Duties:
- Initiates new patient record and repair folders ensuring records are easily maintained and retrievable and accurately files information in the patient record in accordance with departmental guidelines.
- Retrieves and provides patient records or information to CHS health care providers, clinical staff, the patient or other authorized JPS staff, patient representatives, or outside agencies when required or requested and for prior scheduled patient appointments for patient care and non-patient care.
- Identifies authorized requesters and users of health information prior to providing access.
- Accurately and in a timely manner adheres to procedures for chart tracking system and accessing other pertinent information.
- Accurately files and arranges patient records in terminal digit or chronological numerical order ensuring record and information is easily and readily retrievable and assists in finding nursing medical records and information.
- Possesses skills and knowledge to interact with all internal and external customers courteously, exhibiting and providing excellent customer service.
- Answers telephones and provide information regarding medical record/health information management and maintain and monitor supplies and forms necessary to perform duties.
- Adhere to Network, CHS, departmental policies, procedures, Quality Improvement Plan, Code of Ethics and Compliance Plans, legal regulations, and patient confidentiality.
- Performs other related duties as assigned.
Qualifications:
- Required Education and Experience:
- High School Diploma, GED, or equivalent.
- 2 plus years of experience with PC's and typing.
