About our Team: 

We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.


About the Role:

  • Support administrative and operational functions of a care team, as an acting member of that care team and liaison to operations team 
  • Serve as a representative of Cityblock to members, other health care organizations, practices, insurance partners, and other partners in ensuring that our members have the care and assistance they need to thrive. 
  • Work closely with team members to coordinate care as a team on behalf of members. 
  • Serve as the lead for the Care Team Operations Associates and support operations team on design and implementation of workflows 
  • Serve as the lead of Social Planning Committee, designing and facilitating team social events 
  • Lead correspondence with external facilities related to home health, acting as the lead liaison and point of contact between Cityblock and CareFirst, Liberty, HHA agencies, and DHCF 
  • Weekly tasks could include:
    • Lead and manage the training and onboarding of all Care Team Operation Associates 
    • Coordinate home health, insurance, vendors, and other social service assistance services for our members 
    • Connect with insurance, other providers, and other organizations to request and deliver appropriate medical services and records for members
    • Support team meetings with note-taking and follow-ups on all commitments 
    • Coordinate & communicate team calendars 
    • Use member benefits platforms to organize medicaid support and enrollment
    • Monitor member disenrollments and identify retention approaches   
    • Manage logistics (rooms, food, planning) for case conferences, development sessions, meetings and team outings
    • Work with the operations team as it relates to market communication, administrative tasks, and team building/engagement strategies
    • Work with the operations team to facilitate member-spend requests 
    • Other job duties as assigned

How We Define Success:

  • Scheduling and Access: Ensure that there is availability for an upcoming sick visit and regular visit requests, and schedule visits within 24 hours of a request
  • Initial Provider Visit: Ensure members visit a primary care provider within 90 days of enrollment as appropriate  
  • Member Spend: Remain in compliance with member spend guidelines
  • LTSS: Ensure all communication requests related to LTSS services, including agency referrals, are responded to within 24 hours
  • Social Planning: Organize and coordinate one team social event per month. Increase enrollment of social planning committee by at least 1 person per quarter 
  • Member Retention: Review and update member eligibility spreadsheet every 48 hours and identify appropriate outreach/retention strategy based on reason for disenrollment 
  • Training and Onboarding: Facilitate trainings for other CTOAs as they are onboarded, and update the market training calendar on a biweekly basis 
  • Market Communication: Ensure the DC Market Intranet, team calendars, and other market communication materials are up to date and distributed as needed 


Nice to Have, But Not Required:

  • Medical office or hospital environment experience
  • Associate’s or Bachelor’s degree
  • Certification or extended focus on data analysis platforms 
  • Experience and interest in healthcare and social services
