About our Team:

We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with members in the Washington DC market,  in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day. 

About The Role: 

  • You will work in a radically different model of healthcare, leveraging expanded care teams, in-person and virtual care models as well as a proprietary homegrown IT platform for collaborative patient assessment and management, in order to deliver a deeply member-centered, high-value experience of care for individuals with complex needs
  • You will lead highly collaborative teams of clinical and non-clinical care team members, focused on shared-decision making, the equal importance of physical, behavioral and social care, and on building trusted relationships across roles and disciplines 
  • As Medical Director: 
    • Be jointly accountable (alongside the Market General Manager) for clinical quality, member engagement and total cost of care management for an assigned cohort of high and rising-risk members in your market
    • Lead in building and sustaining a culture centered on value-based care delivery, orienting teams towards opportunities to engage members longitudinally and invest in community-based interventions to improve outcomes, in particular reducing avoidable hospitalizations and downstream progression of chronic disease
    • Provide clinical oversight and supervision for advanced practice clinicians and MDs, DO’s, and Pediatricians assigned to the market
    • Play a lead role in monitoring, evaluation, and quality improvement activities, including review of member and care team quality of care concerns
    • Serve as the clinical expert in the care of members with complex needs, and drive the integration of palliative care, primary behavioral health care, and substance use disorder screening and treatment as part of normative primary care 
    • Interface with the larger Cityblock organization in collaborating on, amongst other domains, quality, safety, clinical and operational strategy and initiatives.  
    • Oversee and mentor team members 
  • As a Cityblock Clinician:
    • Co-manage (as part of an interdisciplinary team) a panel of members with complex needs in a value-based care environment to provide diagnosis, treatment, counseling, medication management, acute triage and follow up care
    • Foster lasting and trusting relationships with members and their family members, to assist members in achieving their goals, identifying new needs, and coordinating care
    • Your work will be centered in both our physical hub and in the field - extending out in the surrounding neighborhood and community
    • Your work will be scheduled and unscheduled member visits - you may perform urgent clinical assessments 
    • Interface with partner PCPs, specialists, hospitals, payer Medical Directors, and community based organizations to promote an understanding of Cityblock’s model, facilitate collaboration, and promote effective patient co-management
    • Participate equally in all call responsibilities
    • Foster lasting and trusting relationships with members and their family members, to assist members in achieving their goals, identifying new needs, and coordinating care.
    • Work daily within our custom-built care facilitation platform, Commons 
    • Attend internal & external meetings and maintain professional credentialing and CME standard

Requirements for the Role:

  • You have an MD or DO degree from an Accredited institution.  
  • Board Certified or Board Eligible in Family Practice or Internal Medicine
  • You have at least 5 years leading providers, practices and care teams operating in a value-based care delivery structure
  • You have been accountable for clinical quality and utilization metrics in a data-driven environment
  • You have experience providing clinical services to individuals with co-occurring chronic medical and behavioral health conditions, as well as with complex social needs
  • Understand the unique complexity of caring for individuals who are eligible for both Medicare and Medicaid  
  • Experience working collaboratively with an interdisciplinary care team, and specifically working alongside community health workers or care coordination staff
  • Have interest in serving marginalized populations
  • Proven skills, knowledge base, and judgment necessary for independent clinical decision-making.
  • You are an organized, efficient, independent self-starter and problem-solver, a leader, a strategic thinker, and a mentor, who is excited about the big picture of whole community health 
  • You are excited to use technology to support your work and help drive the ongoing evaluation toward new and better care
  • Have proficient experience using and navigating an EMR

How We Define Success:

  • At thirty days, you will have a foundational understanding of the Cityblock models of  care as well as the core leadership responsibilities for the role, and the key levers for your market’s overall clinical success
  • At sixty days, you will have begun clinical care and supervision
  • At ninety days, you will have established yourself in your leadership roles, inclusive of managing your team, working within your local and institutional leadership structures, and understanding and responding to the data and metrics  
  • By six months, you are comfortable in all current functions of your role within your Cityblock practice, are meeting your goals and driving success for your team

Preferred, But Not Required: 

  • Home-visiting clinical experience preferred
  • Experience in Palliative and Hospice care
  • Understanding of advanced care directives, and the prevalence thereof, will be critical to success in this market...
  • You have experience with quality improvement, monitoring and evaluation, health systems strengthening, innovation and training
  • Understand the principles of aging in place and long-term supports and services
  • DEA-approved to provide buprenorphine treatment for substance use disorder
  • Multilingual
