Provides patient care and/or patient care assistance by performing various and specific roles within the Clinical Health team. May perform support or coordination roles or may work as a Nurse Practitioner. In a support or coordination role, is responsible for administrative and coordinating tasks necessary to increase physician and clinical efficiency and improve the overall patient experience. As Nurse Practitioner, works with physicians and health coaches to offer an excellent patient care experience and clinical outcome. In all cases, responsibilities are first and foremost to ensure the best possible patient care and clinical outcomes.

Responsibilities:

  • Assists patients and the Care Team by providing clinical and administrative assistance.
  • Greets patients and answer phones in a pleasant and professional manner.
  • Schedules appointments, manages medical records, and handles confidential documents.
  • Responsible for daily communications and coordination of patient basic medical needs, including researching, coordinating, and communicating medication refills and referrals.
  • Responds to telephone and email messages in a timely manner; redirecting calls as needed.
  • Assists the Care Team with calls, specialty appointments, and follow-ups.
  • Ensures a smooth patient flow, timely care, and outstanding customer service for patients and their families.
  • Ensures all patient reports, laboratory results, and information are filed in the correct patient medical records.
  • Maintains the confidentiality of patient information and medical records in accordance with Health Insurance Portability and Accountability Act (HIPAA) requirements.
  • Provides medical support by escorting patients to the examination rooms, interviews patients, measures vital signs including: weight, blood pressure, pulse, and temperature; documents all information in patient’s chart.
  • Performs blood draws, administers injections and skin tests.
  • Assists the physician or nurse practitioner with any on-site patient care needs within the scope of training and under the supervision of the primary care physician.
  • Keeps examination rooms stocked with adequate medical supplies. Order additional supplies as necessary.
  • Creates promotional materials for the Medical and Wellness Center events.
  • Promotes the Medical and Wellness Center services at stores and via telephone to groups.

Skills:

  • Demonstrates and applies a basic knowledge of medical care support.
  • Demonstrates an understanding of Health Starts Here and how it applies to patients and their care givers.
  • Recognizes when deviation from accepted practice and procedure is required.
  • Uses innovative solutions to enhance personal work.
  • Good working knowledge of Microsoft Office products, including Word, Excel. PowerPoint, Outlook, and SharePoint.
  • Proficient with office equipment (copier, fax, shipping and receiving tools).
  • Maintains the confidentiality of all patient information.
  • Understands and complies with HIPAA requirements.
  • Applies knowledge and skills to perform tasks and resolve emerging issues.
  • Displays an understanding of Clinical Health’s operations and procedures required to complete daily work activities
  • Responds to requests in a reasonable time frame.
  • Represents the team in a professional manner at all times.
  • Demonstrates strong verbal and written communication skills.

  • Works well as a team member, sharing responsibilities for tasks and objectives.
  • Participates in team meetings and contributes ideas as appropriate.
  • Builds strong internal and external working relationships.
  • Recognizes when novel, creative approaches can improve task completion and outcomes.
  • Works on assignments governed by standard policy and protocol.
  • Resolves routine questions, referring more complex issues to higher levels.
  • Recommends steps for resolution of unexpected or difficult questions / issues.
  • Ensures accuracy of data and information provided to Team Members.
  • Understands and supports WFM’s model of care and medical guidelines.
  • Understands the WFM organizational structure, associated hierarchy, and approval levels.
  • Understands the types and timing of various departmental deadlines and events.

Education & Experience:

  • LVN / LPN / RN license or Medical Assistant Certification obtained and in good standing.
  • Bilingual English/Spanish proficiency required.
  • 2+ years of administrative experience or equivalent in an out-patient primary care clinic setting.
