The Role


Exciting role for a dedicated leader who is at the place in their career where they want to make a difference in the lives of the people entrusted in our care. The Manager of Health Facilitation possesses a strong commitment to providing superior purpose-driven leadership while seizing opportunities to build trusting relationships with all stakeholders (members, providers, staff). The manager will promote and oversee the day to day health facilitation delivery that is culturally inclusive, diversified, and impacts the health equity of our members with the most challenging and complex health issues to help achieve their full health potential.

Who we are looking for

  • Active RN license in NY and OH (Advanced degree in nursing preferred)
  • 5 or more years experience leading complex care management/health facilitation team
  • Strong clinical skills 3 or more years of care management experience in managed Medicaid, Medicare, or private medical practice.
  • Experience working with people with Intellectual or Developmental Disabilities (IDD)

What you will do

  • Collaborate with the leadership team at the Primary Health Locations, the Group Homes serving our members with IDD, and or specific services of an MSO to provide expert solutions for complex medical, behavioral, and social determinants of health issues.
  • Lead the team of nurses, health coaches, social workers, community health workers in Identifying, prioritizing, and resolving issues affecting the delivery of services.
  • Develop and implement policies and procedures to ensure the delivery of empathetic health facilitation. 
  • Participate in creating workflow processes to enhance quality, productivity, seamless collaboration, communication and care coordination with internal and external resources.
  • Facilitate the development of alliances with our members as we deliver culturally sound solutions to the member-centered plan.
  • Educate the health coach staff and health facilitators on techniques for empowering members in self-management, and overcoming barriers.
