About the Role:

The Manager of Care Strategy & Improvement will partner with the General Manager of Direct Contracting to drive strategy and operations for Cityblock’s participation in CMS’ Direct Contracting program. 

Reporting to the General Manager of Direct Contracting, this role will play an integral part in shaping and operationalizing Cityblock’s care model for attributed Medicare Fee-for-Service members. This leader will collaborate with the General Manager and clinical leaders to ensure that all necessary workflows are designed and implemented effectively, that we are measuring our progress accurately and insightfully, and that we are identifying and prioritizing potential opportunities to improve care for members.

This leader will collaborate with Cityblock national operations teams, owning cross-functional initiatives, and ensuring that they are executed thoughtfully and thoroughly. The Manager of Care Strategy & Improvement will also develop professional relationships and independently work with external provider partners with minimal oversight.

Duties will include the following:

  • Develop, own, and  share accountability for market strategy and achievement of key operational results.
  • Lead run rate market operations initiatives for a set of critical functions that could include, but are not limited to: preparing for program performance reviews, identifying and prioritizing strategic initiatives, and budgeting and forecasting.
  • Design and implement strategic initiatives in coordination with clinical and operational stakeholders to improve quality, member outcomes, and total cost of care performance.   
  • Proactively anticipate and address operational issues as they arise, and collaborate with other key team members to ensure that problems and gaps are solved quickly and seamlessly.  
  • Ensure that all learnings from process builds are tightly captured so that we are continuing to develop our care model and implementation playbooks for future markets; implement new performance metrics where they apply.


Requirements for the Role:

  • 6-8 years of healthcare consulting or similar healthcare/management experience.
  • Experience and comfort working within a high-paced, ever-evolving startup environment, working with operators, technologists and clinicians. 
  • Comfort with analyzing data, using a variety of data tools, highlighting key insights, and creating  compelling dashboards and other data visualizations to drive decision making. 
  • You have excellent project/process management skills, and you are rigorous, energetic, agile, flexible, and aren’t afraid to do the work yourself.  
  • Extraordinary oral and written communication skills. 
  • Bachelor’s degree required.
  • Must be based in the District of Columbia metro area and be required to travel up to 20% within the District to external provider partners.


Nice to Have, But Not Required:

  • Similar experience within an innovative care delivery organization and/or high growth medical group or health system
  • Experience with value-based contracts (e.g., ACOs, capitated arrangements), especially CMS’ Medicare Shared Savings Program or Direct Contracting Program.
  • Understanding of population health and how value-based providers drive performance.
