The Role

The Licensed Clinical Social Worker is a key member of the Circulo Health team who cares for the emotional health of our members as a way to ensure the success of the Health Hub.

Who we are looking for:

• Skilled in cognitive behavioral therapy and/or acceptance and commitment therapy, motivational interviewing, and mind-body practices.
• Ready to collaborate with members to establish care plan goals, to utilize the most appropriate and current interventions, to work towards achieving the desired outcomes.
• Excited about collaborating, shared decision-making, and partnering across teams.
• Adaptable, organized, efficient, independent self-starter and problem-solver, a leader, a strategic thinker, and a mentor, who is excited about the big picture of whole community health
• Flexible in an ambiguous and dynamic environment.

What you will do:

• Provide individual and family counseling to support members, their support systems, including families and caregivers.
• Facilitate group-based visits to address medical and behavioral health needs holistically.
• Co-manage the assessment and treatment of psychiatric concerns, psychosocial issues, and health behavior change. 
• Improve population health outcomes by ensuring all patients and primary care team members have access to behavioral health expertise.
• Evaluate crisis situations and employ appropriate interventions.
• Comfortable visiting members in their homes. Ability to travel within the Columbus, OH area to complete home visits. 
• Willing to commute 2-3 days to the Columbus, OH location

Degree Requirements and Certifications

• Master’s degree in social work or a related discipline
• Licensed Clinical Social Worker with licensure in Ohio
• Social work experience in a health care setting, experience within a provider practice is a plus
• Experience with Intellectual and Developmental disabilities is a plus
