Role Description:
 

The Director of Care Services will support a new division in the organization that delivers primary care and support services in patients’ homes. This includes adapting the Oak Street Health primary care model for the home-based setting, as well as developing and operating new service lines to drive improved care outcomes and quality measures for Oak Street patients and for external customers.
 

The Director of Care Services is instrumental in designing, implementing and refining home-based programs.  They will engage functional leaders to aid in strategy and program design by leveraging existing programs, resources and/or experience.  Once programs are ready for execution, they will lead care teams through on-the-ground implementation of new and innovative care models, targeted to the appropriate patients, to improve quality and cost of care. They will be an effective cross-functional team leader, working side-by-side with providers, care teams, operations teams, and corporate teams, with a strong ability to influence and help manage teams through change. 
 

This is a high-intensity, high-visibility role on a fast-growing team. Travel will be required as needed, and may be frequent at times.  The role will report directly to the Senior Vice President of Care Services. 

 

Core Responsibilities:

  • Support the build and growth of a new-to-Oak Street care model to provide longitudinal primary care in patient homes, in partnership with Oak Street clinical experts

  • Support the build a set of services to optimize quality and outcomes for Oak Street patients and as a service to health plans for non-Oak Street patients. These services can include health risk assessments, preventive screenings, care management, etc.

  • Support thoughtful program design to ensure programs deliver consistent, outstanding patient outcomes across the population we manage.

  • Understand home-based business model, economic model, and strategic objectives in detail. Be able to lead teams through these topics. 

  • Own the program implementation with accountability to key program metrics including operational measures, quality measures, utilization measures, and financial measures.

  • Other duties, as assigned 

 

We’re looking for motivated individuals with:

  • 5+ years of applicable experience in a healthcare setting, ideally in a care delivery or health insurance organization working with multidisciplinary clinical teams

  • Strong leadership and influencing skills, with ability to build the trust and confidence of a wide range of stakeholders

  • Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives

  • Quick learner who is comfortable with a high level of ambiguity

  • A problem-solving orientation and eagerness to quickly solve immediate needs, with vision and ability to develop longer-term, sustainable processes

  • Incredible level of organization and detail orientation

  • Analytical and data horsepower to develop business cases, ROI analyses, and financial analyses

  • Pleasantly persistent approach, with a flexible and positive attitude

  • A supportive attitude toward our patient population of older adults

  • A passion for mentorship and team-development

  • Ability to manage multiple priorities in a fast-paced environment

  • Expert in MS Office and Google Apps

  • Bachelor’s Degree required; advanced degree a plus

  • US work authorization
