Description: The Community Resource Specialist functions as an important link between the patients primary care team and their surrounding community and empowers patients to get active in their own care. This role develops relationships with community organizations and connects patients with community resources The Community Resource Specialist uses standardized assessment tools to help determine barriers to patient meeting identified health goals and supports patients change behavior by creation of an action plan for meeting health goals. The Community Resource Specialist also helps patients navigate through the Kaiser Permanente system.

Essential Responsibilities:

  • Supporting patients with low to medium care needs to address identified social health needs, impacting their health, with a goal of removing barriers.
  • Actively linking patients to community resources, using Kaiser Permanente endorsed systems to better meet the patients specific needs.
  • Work with patients around goal setting, action planning, problem solving and follow up. Support may be telephonic, face to face and or via virtual consultations. Document patient interaction and services provided in the electronic medical record.
  • Accepts referrals from all members of the care team, assessing patient needs and provide advocacy as needed for the patient. Prioritize referral work throughout the workday, including electronic and warm handoff referrals, consultations with patients that have complex needs.
  • Developing contacts in the local community and maintaining ongoing relationships within the community. Developing and maintaining relationships with community-based organizations and contributing to the collection of information for an organization data base. Includes attending select community meetings, coalitions and events.
  • Act as an active member of the care team, including educating and informing staff of local resources and collaborating with others across the organization to support patient needs.
  • Navigates the Kaiser Permanente WA system and has knowledge of appropriate internal programmatic referrals.


Basic Qualifications:

Experience

  • Minimum one (1) year customer service experience.



Education

  • High School Diploma or General Education Development (GED) required.



License, Certification, Registration

  • N/A



Additional Requirements:

  • Experience interacting in a diverse, multicultural and/or multilingual environment.
  • HCA Assistor Training (to be completed within 30 days of hire if not present at hire).
  • Community Health Worker training (to be completed within 180 days of hire if not present at hire).
  • Computer literacy including Word, Windows, Excel, Social Media, and Electronic Medical Record.
  • Understanding and sensitivity to how socioeconomic, environmental, cultural, and other factors influence health.
  • Ability to participate and collaborate in a team setting.
  • Good organizational skills.
  • Strong communication skills.
  • Ability to multi-task.
  • Self-motivated.



Preferred Qualifications:

  • Two (2) years of working in a health care or community service organization.
  • Experience with Motivational interviewing.
  • Bachelors Degree.
  • Familiar with and active in local community.
  • Bi-or multi-lingual.
  • Understanding of Medical Terminology.
