Could this be you?

The Care Coordinator is an integral part of a primary care team which shares responsibility to improve quality of care, ensure appropriate utilization of resources, and facilitate a positive experience for patients, families and caregivers. In this role you will support our Nursing and Social Work Care Managers with patient engagement, community resource coordination and aggregating important health information across the continuum of care. The Care Coordinator will work under the direction of our licensed care team members providing education, support and linking to community resources; housing; medication, finance and insurance assistance; and other needed resources.

How you can make a difference

  • Coordinate with local community resources as needed to support both the physical and psychosocial needs of patients
  • Collaborate with multi-disciplinary team members to provide best in class patient care and outcomes
  • Assist patients in applying for local community resource programs such as Social Security Disability, Medicaid, Food Stamps, etc.
  • Gather necessary documents to complete and submit applications directly to the appropriate agency
  • Record pertinent data in required areas to ensure team members can provide care in an efficient, continuous manner
  • Outreach and engage low acuity, high risk patients and screen for Care Management needs
  • Request records from specialists enabling the continuity of care for both PCPs and members of the interdisciplinary care team
  • Travel to patients’ homes when necessary

Experience to drive change

  • Bachelor’s degree required
  • Community Health Worker certification or bachelor’s degree Social Work preferred
  • Previous experience in a social service-related field
  • Strong communication and relationships skills required; demonstrated success partnering with physicians and care teams to support patient wellness
  • Knowledge of community resources for targeted, at-risk, populations
  • Comfort with technology including the Microsoft suite of products
  • Prior experience using electronic health record including data capture, data mining and reporting
