We’re currently looking for an experienced Nurse Practitioner to join our expanding Complex Care Transitions (CCT) program. Developed to support the complex needs of high-risk patients, the VillageMD CCT program is a Nurse Practitioner-led, interdisciplinary care model that aims to stabilize our highest complexity patients in the post-acute setting to ensure a safe, successful, and sustained transition back to their home environment. The program is designed to minimize avoidable readmissions through frequent touchpoints by the clinical team and provide comprehensive, coordinated support in the 30-day post-discharge period.

As a Nurse Practitioner with VillageMD’s CCT program, your main responsibilities will include:

  • Evaluating and treating patients engaged in the program at least weekly using telemedicine.
  • Applying evidence-based guidelines and protocols to manage various conditions.
  • Incorporating a range of remote patient monitoring data to inform clinical decision-making
  • Documenting patient interactions in the electronic health record on day of service.
  • Collaborating closely with other clinicians, pharmacists, care managers, and care coordinators.
  • Consulting with the program Medical Director on individual cases, as needed.
  • Presenting updates on enrolled patients at weekly care conferences.
  • Providing feedback on program design, patient engagement strategies, electronic health record templates, and clinical trainings and protocols.
  • Ability to work weekday shifts between 8:00 am – 5:30 pm.
  • Rotating weekend call coverage may be required.

Qualifications for the Advanced Practice Provider:

  • Graduate of an accredited Nurse Practitioner program with a current NP certification.
  • Three years or more of clinical experience, with the ability to work autonomously. A physician lead will be available for clinical support, as needed.
  • Comfortable treating complex adults aged 65+, conducting comprehensive or episodic health history and physical assessment of patients with acute and chronic conditions.
  • Proficient in review data and documenting in electronic health records.
  • Current, unrestricted license in Arizona and Georgia
  • Eligible and willingness to apply for licensure in other Village Medical markets.
  • Active Drug Enforcement Agency (DEA) registration.
  • Active Medicare/Medicaid Enrollment or be eligible to enroll.
  • Strong oral and written communication skills.
  • Comfortable using Microsoft Office.
